Dashboard

The NCL LTC LCS dashboard has been created to enable practices and PCNs to view and monitor their eligible populations, activity, outcomes, and their overall progression through the Model of Care (MoC).

Please note that there is no Patient Identifiable Data (PID) throughout the dashboard. In the interest of transparency, practice names and activity data is available to be viewed by any NCL practice, which was agreed at a recent LTC LCS Learning, Improvement and Evaluation Task & Finish Group, attended by borough Clinical Leads and ICB colleagues.

Dashboard Registration:

If you encounter any issues with the registration process, please get in touch with our IT and Systems team in the first instance by emailing nclicb.itandsystems@nhs.net.

There may be a slight delay while your registration is approved but you will be notified as soon as you are able to log in. Please note that only NHS.net email addresses can register.

Dashboard Overview:

Once registered, you can select your borough, PCN and practice from the drop-down menus at the top of the screen. The ‘One View’ tab on the left-hand side will show your practice’s achievement to date with the PCN outcomes selected for the LTC LCS, as well as all other PCN outcomes selected in the other boroughs.

You can also view your progression through the MoC and how many eligible patients in each risk group have passed through each stage of the MoC. This data can also be viewed in graph form using the tab on the left-hand side called ‘MoC Progression’.
