Personalised Care Awards London 2026 are now open for nominations
Across London, neighbourhood teams are bringing people and organisations together in new and innovative ways – connecting primary care, community services, local authorities, VCSE organisations, social care, carers and communities to provide care and support around what matters to people.
These awards are an opportunity to recognise and celebrate the people, teams and partnerships making this happen.
Nominations close on 15 October 2026.
