This session is aimed at NCL primary care professionals who work with heart failure patients.
This session will cover:
When to suspect and how to diagnose heart failure
Treatment pathways, including lifestyle reccomendations
Diuretic management vs kidney care
When to refer to secondary care
When to start thinking about end of life care
Community support for heart failure patients and access to cardiac rehab across NCL for heart failure patients

Background:
NCL Training Hub is coordinating with local consultants, primary care leads, specialist nurses and public health to arrange a series of webinars to provide updates on guidance, pathways and resources to support good care for the conditions within the LTC LCS programme. Events are open to anyone across NCL delivering care for patients with long term conditions. All presenters are keen to address learning needs across the patch.
