LTC Update Webinar: Heart Failure

This session was aimed at NCL primary care professionals who work with heart failure patients.

This session covered:

  • When to suspect and how to diagnose heart failure 

  • Treatment pathways, including lifestyle recommendations

  • Diuretic management vs kidney care

  • When to refer to secondary care 

  • When to start thinking about end of life care 

  • Community support for heart failure patients and access to cardiac rehab across NCL for heart failure patients
