Webinar - Supporting Advance and Urgent Care Using the Universal Care Plan: A refresh

The Universal Care Plan (UCP) is a digital personalised care and support plan that enables every Londoner to share ‘what matters to them’ with all professionals involved in their care. 

A UCP can be created following a conversation between health or care professional and the person they care for. The care plan can support advanced and urgent care planning amongst other care pathways including sickle cell disease. This information is then shared digitally in real time with urgent care services to ensure individualised support plans can be upheld and acted upon. 

This event is aimed at primary care health professionals using or looking to use the UCP. We will explain the solution, how to access it and its primary use case providing some education around how to complete a care plan.
