London Personalised Care Awards 2026
We are delighted to announce that the Personalised Care Awards London 2026 are now open for nominations, with the awards ceremony taking place on 3 December 2026 at Governors Hall, St Thomas’ Hospital, London. We were blown away by the incredible nominations received last year - truly inspirational!
Across London, neighbourhood teams are bringing people and organisations together in new and innovative ways – connecting primary care, community services, local authorities, VCSE organisations, social care, carers and communities to provide care and support around what matters to people.
These awards are an opportunity to recognise and celebrate the people, teams and partnerships making this happen.
Is your neighbourhood working differently?
Perhaps your Integrated Neighbourhood Team is:
• Bringing health, care and VCSE partners together around the needs of local communities
• Working across organisational boundaries to improve people’s experiences of care
• Giving people greater choice and control
• Connecting people to their communities and local assets
• Tackling health inequalities and improving access for underserved communities
• Supporting people to live well and remain independent
• Using resources differently to achieve better outcomes for local people
• Co-producing solutions with people with lived experience
• Developing new approaches to personalised care across your neighbourhood
If so, we would love to hear about it!
The Personalised Care Awards celebrate outstanding contributions from individuals, neighbourhood teams, partnerships and organisations that are making personalised care a reality across London.
Who can nominate?
Anyone can nominate work they believe deserves recognition, including:
• Integrated Neighbourhood Teams
• Primary care and community teams
• VCSE organisations and community groups
• Local authority teams and officers
• Carers and people with lived experience
• Nurses, AHPs, social workers and other frontline staff
• Health and care leaders
• Members of the public
You don’t need to have a large project or a huge budget. We want to hear about the difference your work is making to people and communities.
Award categories include:
Working Together Differently | What Matters | Shared Decision-Making | Collaborative Working | Seeing Me | Choice and Control | Giving Confidence | Connecting People | Living in a Digital Age | Health Equity | Using Our Money Well | Green
Whether it is an individual making a difference, a neighbourhood partnership that is working in a new way, or a community-led approach that is improving people's lives, please nominate them and help us showcase the great work happening across London.
