New Brough Level NCL LTC LCS Model of Care sessions

To support practices with the implementation of the Long-Term Conditions Locally Commissioned Service, NCL Training Hub have organised drop-in sessions to provide clinical staff with support to implement stages in the LTC Model of Care.
