The aim of the programme is to support GP practice teams in the management of type 2 diabetes in patients aged 18- 40yrs with insights from specialist consultants on complex cases, practical steps for Clinicians implementing learnings into daily practice and reimbursement incentives for sharing knowledge within teams.
