The NCL Model of Care for patients with or at risk of developing long term conditions

Since 2022 General Practice across North Central London has been supported to adopt a new Model of Care for managing long term conditions (LTCs) through implementation of the NCL LTC LCS. At introduction the NCL Training Hub was commissioned to support practices to deliver the LTC LCS. The NCL Training hub continues to support education related to LTC LCS and hosts the resources which supported practices to launch the programme on this website.

This website contains resources to stay up to date with updates for LTC LCS Training and Support including planned webinars, training opportunities, and published resources.
