Long Term Conditions

The NCL Model of Care for patients with or at risk of developing long term conditions 

Since 2022 General Practice across North Central London has been supported to adopt a new Model of Care for managing long term conditions (LTCs) through implementation of the NCL LTC LCS.

This website contains many resources to support your practice to successfully introduce and deliver the Model of Care.
