Long Term Conditions Locally Commissioned Service Explainer May 2024

This video is 25 minutes long. We recommend using it at induction of new staff so that everyone is familiar with the LTC Model of Care in NCL. It also includes a case study to demonstrate how the Model of Care works for patients.
