Webinars, drop in sessions and short training videos have been produced to support practice teams to understand the Model of Care and the IT resources that support it. Please see the links below. Whilst these training videos still remain relevant, please note some changes in the programme since they were developed. MyCAW is no longer the care plan we use. You will find a simpler care plan embedded in your EMIS templates. The LTC LCS no longer mandates a specific number of follow ups but expects that clinicians will arrange further follow ups as necessary to optimise care for more complex patients.

Long Term Conditions Locally Commissioned Service Explainer May 2024

This video is 25 minutes long. We recommend using it at induction of new staff so that everyone is familiar with the LTC Model of Care in NCL. It also includes a case study to demonstrate how the Model of Care works for patients.
