A series of training events were organised in the first year of the programme, many of these can be found in the recorded training links below. They support both the process of delivering the model of care and the clinical knowledge to care for patients with the included long term conditions
Upcoming Training and Support
NCL training hub will continue in year 2 of the LTC LCS to organise training to help support practices. The current training on offer can be found below. If there is any further training that practices feel is needed then you can feed this back via the nclicb.ltctraining@nhs.net

NCL Long Term Conditions Clinical Update Series
NCL Training Hub has worked with local consultants, primary care leads, specialist nurses and public health to arrange a series of one-hour lunchtime webinars to provide updates on guidance, pathways, and resources to support good care for the conditions within the LTC LCS. These webinars were recorded in 2023/24
Ongoing Training Offers
To provide the Children and Young People Asthma Model of Care you will require clinicians with Tier 3 training for some parts of the model and Tier 2 for others.

Weight Management Training is available through the NHS eLearning for Healthcare platform.
The Physical Activity Clinical Champions are a group of clinicians across the country who are trained to deliver sessions to health care professionals to increase conversations with patients about physical activity levels.
They are funded to offer free training to health professionals ( face to face or remotely ) in sessions minimum 1 hour and max 3 hours in length.
They work in partnership with local public health teams and Active PRactice so that when they present the teaching , they can also signpost to what is available locally to support patients.
Groups should have a minimum of 10 -12 attendees but can be much larger and can be delivered at practice / PCN / borough or NCL level. If you would like to organise a session for your clinical team please email jan.sinclair@stockport.nhs.net
