General Practice Change Management Support Programme

We offer bespoke, hands-on support to help GP practices improve patient access, care for people with long-term conditions and digital transformation - delivered through a local borough team.

What we can support

  • Digital - supporting effective use of digital tools and workflows

  • Long-term condition care - helping you deliver the Model of Care sustainably, embedding personalised care and meeting targets

How it works

Our approach is led by local clinical and non-clinical facilitators, using quality-improvement methods and peer learning that is tailored to you. We help practices find ways of working that reduce pressure and free up time in the long run.

Initially, support will focus on practices that need the most help in these areas, alongside a universal offer available to all practices. This is purely a support offer - not performance management or assurance activity.

Meet the team

The programme is led by a team from Haringey and Islington GP Federations and the Qualitas Consortium, with all NCL GP Federations providing local support and delivery.

The Operations Manual is the central reference guide for delivering the NCL General Practice Change Management Support Programme across North Central London. It sets out how the programme works, who is involved, and how structured, practical support is provided to GP practices and Primary Care Networks to improve access, digital capability, and long-term condition management.
