General Practice Change Management Support Programme

Introduction

The NCL General Practice Change Management Support Programme is designed to help practices:

• Improve patient access

• Enhance care for people with long-term conditions

• Accelerate digital transformation

The programme will run until March 2027.

How the programme works

Practices will receive tailored support from borough-based change teams, backed by an NCL-wide team. This support includes:

• Practical change management and quality improvement expertise

• Opportunities for peer learning

Initially, support will focus on practices that need the most help in these areas, alongside a universal offer available to all practices.

Who’s involved?

The programme has a leadership team from Haringey & Islington GP Federations and Qualitas Consortium. All NCL GP Federations are providing local support and delivery. Meet the team and learn more about the programme.
