Daniela has been nominated for her exceptional leadership in transforming the administrative processes within the practice. Faced with growing workloads and high outstanding task lists, she revamped staff training, introduced robust data monitoring systems, and fostered a positive team culture. Daniela created a bespoke tracking system to identify trends and allocate resources effectively, leading to significant reductions in outstanding tasks. Her hands-on approach and motivational leadership empowered the team, improving efficiency and morale. Daniela’s work exemplifies the power of data-driven leadership and teamwork.

Recognise your own limits and do not be afraid to speak up and ask for help when things are not right. Remember that patient safety and delivering the best care possible is at the forefront of the duties of a GP and whenever the care is being compromised it is your responsibility to speak up and raise this to the relevant people. Practice processes should always be open for review and to be adaptable to change.

Pradeep has been nominated for his exceptional leadership at Carlton Court, focusing on staff development, resident empowerment, and community engagement. His initiatives include personalised living spaces, a Memory Loss unit, and digital care systems, which have enhanced resident well-being and reduced medication errors, falls, and pressure sores.
Pradeep’s outreach efforts, such as intergenerational activities, pet therapy, and community events, have fostered a strong sense of belonging. Pradeep’s leadership has led to national awards for his team, a CQC rating improvement, and double platinum Investors in People accreditation, exemplifying excellence in care and innovation.

Creating a Successful Leadership Team across a Merged PCN
Dr Claire Hassan, Clinical Director for Barnet PCN2, has been nominated for her exemplary leadership in uniting 12 practices to create the largest PCN in Barnet. Her mentorship has fostered a highly effective leadership team that has driven projects addressing health inequalities and improving patient care. Achievements include the Cervical Screening Project, which increased smear uptake among BAME groups and beyond, and the Blood Pressure Case Finding initiative to improve hypertension
detection. The team also developed a Proactive Housebound Review Pilot and tackled digital exclusion. Successes such as care navigation training, enhanced learning disability reviews, and a FIT test pilot highlight their dedication to innovation, collaboration, and equity in healthcare delivery.

With a large partnership model of 11 GPs and a managing partner, the practice invests in personal development plans, mentorship programmes, and tailored training. Success stories include Sujitha, a receptionist mentored into becoming a Health Care Assistant, now delivering phlebotomy services, and Yeliz, a receptionist supported to realise her
